DS-260 Immigrant Visa Form: Guide to the Consular Process – Part II

Due to the volume of information required, completing the form DS-260 is time-consuming and requires careful attention. Collecting your personal information in advance will make the process much easier. You may access a DS-260 sample provided by the U.S. Department of State here: DS-260 SAMPLE

Common examples of information requested include:

  • Full legal name

  • Whether you have ever used another name (for example, maiden name or married name)

  • Nationality or nationalities

  • Passport number

  • Residential addresses since your 16th birthday

  • Phone numbers used over the last five years

  • Email address

  • Social media accounts (Instagram, Facebook, etc.)

  • Mailing address

  • U.S. mailing address (where your green card will be sent)

  • Information about your parents, including full names, dates of birth, and cities of birth

  • Information about your spouse and children (if applicable)

  • Information about prior marriages (if applicable)

  • Etc.

⚠️ Important: The information provided may be verified and cross-checked against other data and documents submitted by the applicant.

The form may be completed in stages, and it is strongly recommended that you save each page periodically. The system may automatically log you out and require you to sign in again, and any unsaved information will be lost.

At the end, carefully review your answers—preferably at a later time or on another day—before submitting the form. Be sure to save a copy of each completed page for your records.
